Chronic Lyme disease — also called post-treatment Lyme disease syndrome (PTLDS) or chronic Lyme syndrome — describes a clinical entity in which patients continue to experience significant symptoms months to years after antibiotic treatment for confirmed or suspected Borrelia infection. Symptoms include debilitating fatigue, migratory joint pain, cognitive dysfunction ("Lyme brain"), peripheral neuropathy, and autonomic dysregulation.
The underlying mechanism remains debated, but evidence points to persistent immune dysregulation, neuroinflammation, autoimmune-like pathology triggered by Borrelia antigens, and potential microbiome disruption from prolonged antibiotic courses. Many patients cycle through antibiotic retreatment with limited durable benefit.
MSC therapy is being explored as an immune-resetting strategy for chronic Lyme — targeting the inflammatory and dysregulated immune state rather than the organism itself.
MSCs modulate the dysregulated innate and adaptive immune response characteristic of chronic Lyme — suppressing overactivated macrophages, restoring Th1/Th2/Th17 balance, and expanding Treg populations that can dampen the autoimmune-like response implicated in symptom persistence. Neuroinflammation driving cognitive and neuropathic symptoms is also targeted through anti-inflammatory paracrine signaling.
While large randomized trials in PTLDS are nascent, patients treated in clinical programs report significant improvements in fatigue scores, cognitive clarity, and neuropathic pain. Several treating physicians have observed normalization of inflammatory cytokine panels and improvements in NK cell and lymphocyte function following MSC infusion.
