Degenerative disc disease describes the progressive loss of nucleus pulposus hydration, annulus fibrosus integrity, and disc height that occurs with aging, mechanical loading, and genetic predisposition. When disc degeneration is symptomatic, it produces chronic axial back pain, reduced spinal mobility, disc herniation, and nerve root compression causing radiculopathy.
DDD is one of the leading causes of disability worldwide, affecting over 650 million people with chronic low back pain. Standard management — physiotherapy, NSAIDs, epidural corticosteroids, and spinal surgery — addresses pain and decompression but cannot reverse disc degeneration or restore nucleus pulposus architecture.

The nucleus pulposus is an avascular tissue inhabited by notochordal cells, and it is notoriously difficult to target therapeutically. When injected intradiscally, MSCs can differentiate into nucleus pulposus-like cells and secrete collagen II, aggrecan, and water-binding proteoglycans that partially restore disc hydration and height. Anti-inflammatory cytokines suppress the discogenic pain cascade driven by IL-1β, TNF-α, and MMP activity.
Randomized controlled trials and prospective cohort studies of intradiscal MSC injection have documented improvements in VAS pain scores, Oswestry Disability Index, and disc height on MRI at 12 and 24 months — with some patients avoiding planned spinal fusion procedures.
