About the Condition

Non-alcoholic fatty liver disease encompasses a spectrum from simple hepatic steatosis (NAFL) to non-alcoholic steatohepatitis (NASH) — characterized by inflammation and hepatocyte injury — and progressive fibrosis leading to cirrhosis and hepatocellular carcinoma. NAFLD affects approximately 25% of the global population and is the fastest-growing indication for liver transplantation.

The pathogenesis involves insulin resistance, lipotoxicity, gut microbiome dysbiosis, and an amplifying hepatic inflammatory cascade mediated by activated Kupffer cells and stellate cells. No FDA-approved pharmacological therapy for NASH exists, and current management relies on lifestyle modification — which is effective but difficult to sustain.

MSC therapy targets the hepatic inflammatory and fibrotic cascade directly, offering a regenerative complement to lifestyle and metabolic optimization for NAFLD/NASH patients.

How MSC Therapy May Help

MSCs administered intravenously home to inflamed hepatic tissue, where they suppress Kupffer cell activation, reduce hepatic NF-κB signaling, and secrete HGF, IL-10, and TGF-β-counteracting factors that inhibit hepatic stellate cell activation and collagen deposition. This anti-fibrotic, anti-inflammatory paracrine effect has been demonstrated in multiple rodent NASH models.

Clinical series in NAFLD/NASH patients have documented reductions in AST, ALT, GGT, and liver stiffness (measured by fibroscan) following MSC infusion, alongside improvements in fasting insulin and BMI. Histological improvement — including reduction in NAFLD Activity Score and fibrosis stage — has been reported in several biopsied patients at 12 months.
