Peripheral neuropathy encompasses a broad category of disorders in which peripheral nerves — those outside the brain and spinal cord — are damaged. Common causes include diabetes (diabetic peripheral neuropathy), chemotherapy-induced damage, autoimmune disease, chronic alcohol exposure, and idiopathic causes.
Symptoms range from burning pain, electric shock sensations, and allodynia, to numbness, proprioceptive loss, muscle weakness, and autonomic dysfunction. Severe cases cause significant functional limitation, fall risk, and reduced quality of life. No pharmacological therapy currently regenerates damaged peripheral nerves.
MSC therapy is being evaluated as a regenerative option for peripheral neuropathy — particularly diabetic and idiopathic forms — with growing clinical evidence supporting nerve repair and symptom improvement.
MSCs secrete neurotrophic factors including NGF, NT-3, and GDNF that directly stimulate peripheral nerve fiber sprouting and Schwann cell activity. In animal models of diabetic neuropathy, intravenously administered MSCs have restored intraepidermal nerve fiber density, improved nerve conduction velocity, and reduced pain behavior.
In human clinical series, patients with diabetic peripheral neuropathy treated with MSC infusion have reported significant improvements in VAS pain scores, vibration perception threshold, nerve conduction velocity, and proprioception — with benefits observed at 3 to 12 months post-treatment.
